A health risk assessment is a questionnaire about a person’s health habits, health history and health numbers. The answers help a person and a health care provider find risks and make a plan to prevent disease. For example, Medicare asks people to fill out a “Health Risk Assessment” at the yearly wellness visit, and Medicare pays for that visit when the provider accepts assignment.
Walgreens, the national pharmacy chain, offers health services in its stores. Its health services page lists in-person provider visits for screenings, health support and in-store clinic visits, along with preventive care, vaccines, and COVID-19 and flu tests.
A similar free tool is the RealAge Health Risk Assessment. See more tools in the wellness resources list, such as My Life Check and MyHealthfinder.
