The CDC Worksite Health ScoreCard is an assessment tool from the Centers for Disease Control and Prevention. It helps employers see whether they use evidence-based strategies to support the health and well-being of their workers.
The ScoreCard asks yes-or-no questions about programs, policies, environmental supports and health plan benefits. The January 2019 manual listed 154 questions in 18 topic areas. They cover five kinds of strategy: lifestyle counseling, supports in the work setting, health policies, health plan benefits and other worksite programs. Each strategy earns points based on the strength of the evidence behind it. The score shows an employer where its gaps are and lets it track progress from year to year.
CDC lists employers, human resources and benefits staff, wellness staff and occupational nurses as users. State and local health departments can also use it. The online version gives automatic scoring, keeps a record of each yearly ScoreCard, and compares a worksite with other registered worksites of the same size. An employer sets up an administrator account and an account for each worksite.
CDC updated the manual again in February 2025. The current version and the online tool are on the CDC ScoreCard page.
