Information for the parents/guardians of children who will be:

• Entering school in the Fall of 2014
• Entering 7^th and 12^th Grades in the Fall of 2014
• Entering college in the Fall of 2014

You may bring your child and a copy of their immunization record to one of the Health Department’s walk-in clinics listed below:

Wednesday, August 6, 2014 8:00am – 4:00pm
Thursday, August 7, 2014 8:00am – 7:00pm
Friday, August 8, 2014 8:00am – 4:00pm
Monday, August 11th 8:00am – 4:00pm
Tuesday, August 12th 8:00am – 4:00pm
