Our goal is to help the patient and family to manage this time of illness with as little discomfort and cost as possible. Home Health can reduce the length of stay in hospitals and nursing homes by continuing intermittent skilled care in the confort of your home. This care also reduces the risk of having to return to the hospital for complications. We are Medicare certified. We have been serving the Home Health needs of this area since 1920. Let Us Care for You! Who is Eligible? Patients of all ages, who are homebound, have a skilled need for nursing, physical or speech therapy. They must only need intermittent skilled care and usually must have an available caregiver. Homebound definition - (For Medicare/Medicaid) - Confined to home. If you have a condition which makes it difficult for you to leave your home or leaving your home is a medically difficult and taxing effort. When Are Services Available? 8:00am - 4:00pm (Monday - Friday) Nurse on call 24 hours a day. Skilled Nursing ( Catheter Care, Injections and IV Therapy, Medication Supervision, Ostomy care, Pain Management, Patient and Family Education, Skilled Assessment, and Wound and Decubitus Care ) Home Health Aides - Bathing, shaving and shampooing, available while receiving skilled nursing, physical therapy, occupational therapy and speech theapy Physical Therapy - Individualized programs developed and designed to help the patient to improve mobility (i.e. walking), strength and range of motion. Speech Therapy - Individualized program developed to aid the patient to omprove communications speech/swallowing skills. Occupational Therapy - Individual programs developed to assist the patient to improve daily living skills and regain independence. Medical Social Worker - Works with the patient and family to assist them in solving social, emotional, and financial problems related to illness. Refers to available community resources as needed. How is Referral Made? Patient must be under care of Physician who is willing to give Home Health orders. How to Pay for Services? - We bill Medicaid, Medicare and most Insurance Companies. |
