What is the Merck Vaccine Assistance Program?

This is a program to provide Merck vaccines free of charge to eligible patients.   Only a $15 administration fee will be charged.

What vaccines are covered?

You may click on any of the above vaccine names to view the Vaccine Information Statements (VIS) about each vaccine.  Each VIS will give you more information about each disease or virus, each vaccine, who can receive the vaccine, the risks and possible side effects.

What are the income guidelines?

Your annual household income (all members of the family) must be less than:

However, Merck recognizes that sometimes exceptions need to be made based on a patient's individual circumstances. Individuals who do not meet these criteria may still qualify for the vaccine program if the patient has special circumstances of financial and medical hardship.  Please note that if an exception is made based on income, the maximum income limit for an exception is $43,320 for individuals, $58,280 for couples, $88,200 for a family of four.

What do I need to do?

It’s simple.  First, call 304-348-8080 and schedule an appointment.  Then, you must complete a Merck Vaccine Assistance Application.  For pre-approval, you can either bring it or mail it to our office at 108 Lee Street East, Charleston, WV 25301 or fax it to 304-346-4756 prior to your appointment.  Otherwise, you will complete the application the day of your appointment.  There will be an approximate 15-minute wait while your application is approved by Merck.  If you will be receiving a vaccination that requires a multi doses (such as Gardasil, Hepatitis A or Hepatitis B), a new application will need to be completed prior to each dose.
