Kanawha Charleston Health Department is a fee-for-service agency.  This means that we can bill certain insurance companies for services rendered.  We are current providers for and can bill the following companies:

Medicaid
Unicare Medicaid
Carelink Medicaid
PEIA
Carelink Coventry
Blue Cross/Blue Shield (BCBS)
CHIPS

Influenza/Pneumococcal vaccine only:  Medicare, Secure Horizons, Advantra Freedom, UMWA, Human, Humana Gold, Wellcare, and American Progressive.  Please bring your PRIMARY insurance card with you to your appointment.

The Kanawha-Charleston Health Department bills only the insurance companies named above.  Submission of insurance information does not guarantee that the insurance company will cover the above vaccines.  If the insurance company does not cover the vaccines, the parent or legal guardian will be responsible for the charges.  If you have insurance other than those listed above, payment must be made by check or cash, at the time of service.  A receipt will be issued to you to submit to your insurance company.  We strongly recommend that you contact your insurance provider for reimbursement information.

Note: For vaccine coverage, eligibility will be verified in advance for those covered by Blue Cross Blue Shield.
